Something Different ...

Employees who are enrolled in the Harris County medical plan can earn the new Healthy Actions Incentive. This year the Healthy Actions program is a monetary incentive that eligible employees may voluntarily participate in. You just need to do a few actions to earn up to $175.
GOAL 1: REQUIRED ACTION

Earn $25 by completing the following action:

Complete your Cigna Online Health Assessment - $25

What is it?

  • An easy online questionnaire that asks questions about your overall health
  • Takes about 15 to 30 minutes to complete
  • Asks basic information like your weight, blood pressure and cholesterol levels
  • Analyzes your answers and gives you a personal wellness score

How do I complete this action?

  1. Visit myCigna.com and register by clicking “Register Now” or log in to your account
  2. Once registered, log in to your myCigna account
  3. Once logged in, roll your mouse over the “Wellness” tab at the top right of the page and click “My Health Assessment”
  4. Complete the health assessment
    1. You must complete the health assessment every year (between November 1 – October 31) to qualify for the Healthy Actions Incentive
    2. If you have previously taken the health assessment, click on “Retake my Assessment”
  5. You will know you have successfully completed the health assessment when you receive your personal wellness score
GOAL 2: YOUR CHOICE ACTIONS

Earn up to $150 by completing additional actions.

Complete your annual physical - $25

What is it?

  • A yearly preventative exam with your primary care physician to discuss your overall health and well-being
  • Also known as: preventive exam / check-up

What can I expect?

  • At your annual physical, your primary care doctor will likely:
  • Ask about your current health, family history, past illnesses/surgeries and chronic conditions to assess your health risk
  • Conduct a physical exam – may include checking your vital signs, listening to your heart and lungs with a stethoscope, checking your ears, eyes, throat, skin, abdomen etc.
  • Conduct screenings specific to your age and gender
  • Make recommendations for a healthy lifestyle

How do I complete this action?

  • Find a primary care physician you trust – by seeing the same person over time, your health care is more likely to be consistent and coordinated
  • Book an appointment for an annual physical exam
  • Make sure to use your Harris County Cigna medical insurance to pay for your annual physical exam – your copay should be $0
  • In order to receive credit for the Healthy Actions Incentive, your doctor’s office needs to code the visit as an annual physical exam

Complete your biometric screening (includes a cholesterol and/or glucose measurement) - $25

What is it?

  • A biometric screening measures your health numbers and can include your weight, blood pressure, cholesterol and glucose levels. It is important to know these numbers to assess your risk for heart disease.

How can I complete this action?

  • Receive an Routine Annual Physical
  • If you regularly have labs done, have your physician complete the Wellness Screening Form

Get a flu shot - $25

What is it?

  • A flu shot is an injectable vaccination that reduces your risk from getting influenza (the flu). It is important to get the flu shot every year, as the vaccination changes yearly.

  • Getting a flu shot is FREE for any Harris County employee covered on Harris County insurance (Cigna).

Get a mammogram (preventive exam) - $25

What is it?

  • A mammogram is a preventative exam to assess for early stages of breast cancer. This screening test is used to find cancer in people who have no symptoms, giving you the chance to treat cancer early while it’s small and before it has spread.

How do I complete this action?

  • Talk to your primary physician to schedule an appointment to receive the mammogram

How much does a mammogram cost?

  • All full-time Harris County employees covered by Harris County medical insurance (Cigna) can receive a mammogram with an in-network provider for a $0 copay, annually.

Get my annual OB/GYN exam (preventive exam) - $25

What is it?

  • An OB/GYN exam is a preventative exam screens reproductive health and can include a breast examination, a pelvic examination and a Pap smear.
  • Also known as: Well Woman Exam

How do I complete this action?

  • Speak to your primary provider or gynecologist to schedule a OB/GYN preventative exam
  • If you complete this action at the same time with your annual physical, ensure that you provider sends separate claims for each action to gain credit towards your Healthy Actions Incentive

How much does a OBG/YN exam cost?

  • All full-time Harris County employees covered by Harris County medical insurance (Cigna) can receive an OB/GYN preventative exam with an in-network provider for a $0 copay, annually.

Get a colon cancer screening (preventive exam) - $25

What is it?

  • A colon cancer screening is a preventative exam that assess for abnormal growths in the colon or rectum.
  • Screening tests are used to find cancer in people who have no symptoms, giving you the change to treat cancer while it’s small and before it has spread.

How can I complete this action?

  • There are several testing options, speak to your primary physician about which testing option works best for you.

How much does a colon cancer screening cost?

  • All full-time Harris County employees covered by Harris County medical insurance (Cigna) can receive a colon cancer screening with an in-network provider for a $0 copay, annually.

Get a prostate cancer screening (preventive exam) - $25

What is it?

  • A prostate cancer screening is a preventative exam that checks for abnormalities such as bumps in the rectum. It helps detect prostate cancer at early stages and reduces the incidence of death from prostate cancer
  • Screening tests are used to find cancer in people who have no symptoms, giving you the change to treat cancer while it’s small and before it has spread.
  • Also known as: prostate-specific antigen (PSA) test

How do I complete this action?

  • Speak to your primary physician about scheduling and receiving a prostate cancer screening.

How much does a prostate cancer screening cost?

  • All full-time Harris County Employees covered by Harris County medical insurance (Cigna) can receive a prostate cancer screening with an in-network provider for a $0 copay, annually.

Complete a routine vision exam (self-reported) - $25

What is it?

  • A routine vision exam is a screening to assess vision, check for eye disease, and updating glasses or contact lens prescription.

How can I complete this action?

  • You must self-report your vision exam by logging into myCigna.com to receive your incentive for this action.
  • The routine vision exam may be completed with any form of insurance including your Harris County vision insurance, Davis Vision. See an in-network provider to schedule a routine vision exam!

How much does a routine vision exam cost?

  • All full-time Harris County Employees covered by Harris County vision insurance (Davis Vision) can receive a routine vision exam with an in-network provider for a $10 materials fee/copay, annually.

Complete a routine dental cleaning (max. 2) - $25 per cleaning

What is it?

  • A routine dental cleaning is used to maintain healthy gums and teeth.
  • Routine dental cleanings remove the buildup or tartar and bacteria that accumulates between gums and teeth.

How do I complete this action?

  • Schedule a routine dental cleaning with an in-network dental provider.

How much does a routine dental cleaning cost?

  • All full-time Harris County Employees covered by Harris Count dental insurance (Cigna) can receive two routine dental cleanings (every six months) with an in-network provider for a $0 copay, annually.

Complete a County-coordinated Mental Health First Aid training class (8 hours) - $50

What is it?

Mental Health First Aid teaches you how to identify, understand and respond to signs of mental illnesses and substance use disorders. This 8-hour training gives you the skills you need to reach out and provide initial support to someone who may be developing a mental health or substance use problem and help connect them to the appropriate care.

  • The virtual Mental Health First Aid Course is free for all full-time Harris County Employees covered by Harris County medical insurance (Cigna)

Complete 9 lessons in the Cigna Diabetes Prevention Program (Omada) within a 26-week period (eligible once per lifetime) - $100

What is it?

The Cigna Diabetes Prevention Program (Omada) is a 16-week online program that inspires healthy, long-term habits. It combines behavior change science and unwavering support that participants need to lose weight and reduce their risk of Type 2 diabetes and heart disease. Omada is personalized to help you reach your health goals – whether that’s losing weight, gaining energy or improving your overall health.

The program includes:

  • A full-time health coach to keep participants on track
  • A wireless scale to monitor progress
  • An interactive program that adapts to the participant
  • Weekly online lessons to educate and inspire
  • A small group of participants for real-time support

How do I complete this action?

 

How much does the Omada for Cigna Program cost?

  • Omada for Cigna is free for all full-time Harris County employees that are insurance eligible and are qualified and accepted into the program.

Complete 9 lessons in the Omada (Diabetes or Hypertension) program within a 26-week period (eligible once per lifetime) - $100

What is it?

Omada for Diabetes is an accredited diabetes self-management and education (DSMES) program that matches certified diabetes care and education specialists to guide participants through personalized curriculum. Omada for Hypertension addresses challenges for individuals dealing with hypertension and assists them in developing behaviors that improve blood pressure control.

How do I complete this action?

  • To earn the Healthy Actions Incentive, you must complete at least 9 lessons in either the Omada Diabetes or Hypertension program within the deadline to complete this action.
  • To participate, you must qualify for the program.

How much does the Omada Diabetes or Hypertension program cost?

  • These programs are free for all full-time Harris County employees that are insurance eligible and are qualified and accepted into the program.

Achieve a health goal by working one-on-one with an Evernorth Health Coach (4-week minimum) - $100

What is it?

  • Evernorth health coaching offers support to reach health goals for all Harris County Employees. You have access to nurses, dietitians and clinicians who will listen, understand your needs and help you find solutions- even when you don’t know where to begin.

How do I complete this action?

  • To earn points towards the Healthy Actions Incentive you must create a health goal with your health coach and achieve it. The goal must include a minimum of 4 weeks working with the health coach.

Complete the 6-week Fit Fam Fitness Challenge - $50

What is it?

A six-week fitness challenge that encourages employees to complete fitness activity in a friendly competition amongst coworkers.

How does it work?

  • Join as a team or as individual
  • Log your activity minutes – sync an available fitness app or manually enter your time
  • Track your progress and challenge friends or colleagues
  • Chat with and cheer on your teammates on the interactive message board
  • Earn badges to mark your achievements
  • Earn a prize for finishing the challenge!

Complete a Harris County Wellness Series (3 classes total) - $50

What is it?

Harris County Employee Wellness offers several wellness series options that are available to all Harris County Employees.

  • Prudential Financial Wellness Series (3 classes total)
    This 3-part series will help you develop and utilize skills in understanding the fundamentals of financial wellness, thinking long term and preparing for retirement and building a solid plan for the future.
  • Beating Burnout Series (3 classes total)
    Everyone gets stressed out once in a while. But if you’re always feeling stressed or can no longer keep up with your responsibilities, it could be job burnout. In this seminar, you’ll find out what causes job burnout, discover how to recognize it within yourself and learn the steps you can take to make sure it doesn’t happen to you.
  • Tobacco Cessation Series (4 classes total)
    Smoking and other forms of tobacco use can be an incredibly unhealthy habit. But it’s also one that’s tough to break. In this series of 4seminars, we’ll do all we can to help you finally kick the habit. Get the full story on all the health problems tobacco use causes. Learn why it’s so hard to quit. Find out which programs and medications can help you succeed. And walk out with your own personal “quit plan.”

All preventive visits and screenings must be completed using your County insurance — Cigna for medical and dental actions. Please allow 60 days for your actions to be recorded. The only preventive care action you may self-report is your vision exam. Please do this on myCigna.com.
